Request for Bids Harbor Square Building 5 Reroofing Project

June 3, 2008

Dear Bidder:

This letter is a Request for Bid for the Harbor Square Building Number 5 located at
170 West Dayton Street. The Port recommends that you attend an on-site walk through on June 11, 2008 at 10:00 a.m. in front of Building Number 5. Staff representatives will be available for any questions that you may have concerning the reroofing project.

Bidders must be a registered contractor, have a current UBI number, have current industrial insurance coverage, have current employment security coverage, be current on state excise taxes, and not disqualified from bidding.

This is a prevailing wage job. Compensation also includes any filing fees for prevailing wage paperwork.

Each bid submittal must include the following documents. If the bid submittal does not include the following documents, your bid will not be considered as responsive.

1. Bid Submittal Sheet No. 1 and Bid Submittal Sheet No. 2
2. Bid Bond (different from Payment and Performance Bond)
3. Non-Collusion Affidavit
4. Anti-Discrimination Certificate
5. Certification of Equal Employment Opportunity Report
6. Three (3) references of similar work completed within the last three (3) years written on a separate sheet of paper.

Successful Contractor shall furnish a duly executed payment and performance bond upon a form acceptable to the Port within ten (10) calendar days following receipt of the Notice of Award. A Performance Bond guarantees the faithful performance of the contract and payment (payment bond) of materials and labor by the contractor to all subcontractors and material suppliers. The bond is submitted by the winning bidder upon award of the contract. The Performance & Payment Bond are typically issued together, as they are so closely related. Bidders should include the cost of the performance and payment bonds in their bid.

Certificate of Insurance, in a form satisfactory to the owner shall be filed with the owner prior to the commencement of Work on this project. Bidders should include the cost of naming the Owner as additional insured, if any, in their bid.

The successful Contractor and each separate Subcontractor shall purchase and maintain such insurance as will protect it from claims under Workmen’s Compensation Acts, Longshore and Harbor Workers Coverage’s as required by law, and other employee benefit acts from claims for damages because of personal injury, including death, and from claims for damages to property, which may arise out of or result from the Contractor’s operations under this Contract, whether such operations be by the Contractor or by any Subcontractor or anyone directly or indirectly employed by any of them. This insurance shall be written for not less than the limits of liability specified in the General Conditions of the Contract, or required by Law, whichever is the greater, and shall include contractual liability insurance as applicable to the Contractor’s obligations under this Contract and shall name the Owner as additional insured.

Bid packets are due and shall include Bid Submittal Sheet No. 1 and Bid Submittal Sheet No. 2 by 5:00 p.m. on June 20, 2008. Bidders can mail or hand deliver bids to:
